What kind of documents does Wellington require for tournament competition?

At the start of the school year, students who are part of the Wellington Debate and Forensic Team will receive a packet of required paperwork. This entire packet must be signed (and several must be notarized) in order to attend and compete at any and all tournaments (including any within Palm Beach County). These documents include:

  • A signed and notaraized CONTRACT;

  • For novices in the classroom, the DEBATE 1 COURSE SYLLABUS spells out information students and parents need to know about the class structure, events, rules, etc.;

  • A signed and notaraized MEDICAL CONSENT FORM;

  • A photocopy of their MEDICAL INSURANCE INFORMATION;

  • A MEDIA CONSENT FORM (so that names and photos can appear on this web site); and

  • A PERMISSION SLIP signed by a parent/guardian.

    In addition, parents are not allowed to drive any students other than their own children UNLESS they have completed a Temporary Duty Elsewhere TDE FORM and provided a photocopy of their AUTOMOBILE INSURANCE to Mr. Gaba.

    Students may not drive to tournaments outside of Palm Beach County.

    For adults who chaperone at away tournaments, TDE forms will be supplied by Mr. Gaba. All adult volunteers must also complete the VOLUNTEERS IN PUBLIC SCHOOLS (VIPS) authorization on-line, whcih can only be done in the Wellington High Schol Media Center. This means all adults who are slated to judge, chaperone, help when hosting tournaments, or any other volunteer aspect associated with the debate program.
